I need an orthopedic surgery.
I retired early and do not have insurance coverage.
I can move and establish residency any where in USA.
It is my understanding that some states like MA and NY require immediate coverage of pre existing conditions.
Also looked into the Obamacare pre-existing insurance.
Looking for good quality insurance coverage where I can choose doctor.
Any suggestions?

Did you took a serious look at health insurance risk pools, which have been created with the sole purpose of treating people with the preexisting condition?

To "establish residency" normally means 12 months of continuous residency -- not simply moving into the jurisdiction. So your plan to establish residency anywhere in the USA falls on its face for that reason.
If you "retired early" you must have some resources. Use them to pay for the surgery you need. ANY doctor will give you a CASH discount. Hospitals will, too. Anywhere from 40% to 60% can be expected -- since this will be the same or more than they get from an HMO or PPO for the same procedure.

I liked the idea of high-risk pool. Most of them require you to be without insurance for 6 months or be denied insurance. The premium depends on your income level based on the previous years tax return. Each state is different, and not all states have high-risk pools. What state are you in? I imagine that is the reason you want to move to a different state? If you have pending surgury most insurers are going to give that a straight decline.

I know the Texas High Risk Pool will take people that were turned down but covering existing conditions is another thing. They will not pay for existing until a waiting period of 2 Years is over, maybe more. There goes the rug from under your feet again!
