Car crash and insurance policy

I got into a car crash last week, I forget to exchange information with the driver ( my first accident and scare) and it was the guy fault. The police give me the case # of the accident. How can I obtain the guy policy number and name from the police report?

It 'should' be on the report..if not call the records dept at the police dept (where you pick up or buy the report) ask them where it is located on the report, if for some reason it's not on there, ask if the police will contact the driver/owner for this info...if they won't you will need to call the person (their phone number should be on the report too) and ask who their ins is with etc....also would be a good idea to let your carrier know their has been an accident, the can assist you with getting this info as well.
