There are usually a couple of options available in most (but not all) states. If their state has risk pool insurance this is usually the best option and in most states has pretty extensive coverage. If their state does not then their only other option is Guaranteed issue limited benefit plans, mini med plans, and discount plans. Well, the applicant has to meet severe qualifying criteria in order to qualify for the Medicare benefits. And, things may go worse if you have disabilities that are hard to determine, such as-disabilities associated with back injuries.
