I think we can try to answer your query if you can be a little more specific. I mean, please tell us if you have taken the services of 'a physician' as mentioned and also if that physician is listed with the insurance company you bought your policy from. If the physician is listed with your insurer and you have taken his/her service, then the physician can very well bill your insurance company directly. In such a case there is no fraud involved unless the physician is billing your insurance provider for services that you have not taken.
