the reason I have to use leads for medicare advantage is that cms prohibits door to door sales, calling numbers on the do not call registry,
and I dont think the phone book lists peoples ages.
I am starting to get referrals from the original folks I signed up for the program, and the initial money I spent on lead generation is paying off in droves.

I am in P&C and we use a couple different avenues. First we do name recognition ads and have to to compete with the big names. A big thing that we do is sponsorship events, ie; bike shows, car shows, fairs, etc. With doing this we typically have a drawing for something fun. From these drawings we have been creating our mailing lists. Along with these mailing lists come our quarterly newsletters to our current cients that includes mentioning constantly about our referral program.
I ran a questionnaire to every single customer and the major influence in making a purchase was resoundingly Word of Mouth. Of the 63% returned, 86% of that 63% stated Word of Mouth as their major influence when making a purchase compared to TV, Radio, Print, Billboard and Direct Mail. Work on your name recognition and make sure your "reputation precedes you" and you will see results.
