Why the license was revoked? Its a important factor to determine whether or not you would be able to reinstate the license. I think one is allowed to restore the license after one year of revocation. But you should check it with the state insurance department.

Or, one is allowed to reapply for the license on the expiration of the suspension period determined by the superintendent.
