Is that a health insurance program offered by an insurance carrier? Or is it a program offered by any bank/credit card company?

May be you should call up the insurer and let them know that you want to cancel the coverage. The representative can help you with the process of policy cancellation.
However, if you write to the carrier, send the letter through registered post and preserve the receipt for future references.
