When do a medical insurance cover dental implants?

Could you also advise me which medical insurance cover dental implants and in what cases do they cover them?

If covered, it would only be in the case of physical injury or congenital deformity that results in the need for "medically necessary" treatment. The need for teeth could be explained by the necessity of being able to chew food for nourishment. But since there are many liquid nutrient products, it might not be as easy to prove as it once was.

But wouldn't they consider the congenital deformity as a preexisting condition?Would they cover that?
Could you be more specific about the type of physical injury we are talking about? I would like to know more about these cases of Physical injury?What are the requirements you need to meet so that the medical insurance would cover the dental implants. Could you please advise me where can I find more information about this type of coverage. thank you very much for your answer.
