what form can i fill out to give my consultant permission to speak with my home insurance

Power of attorney.

I guess tcope is pointing at a financial power of attorney which has to be given to someone who's well aware of the state rules and guidelines. This individual will be responsible for handling financial transactions, sign agreements, property selling etc. He'll have to be someone who can protect your interests.
