an insuranse company have added a person on my policy without my knowledge

I can't know for sure... but I doubt that they did. No they cannot do that.

If you admit that a previously unlisted driver (especially if they had the accident) regularly uses the insured vehicle, then may add them to the policy through your statement.
Remember, you agree to tell the insurance company about ALL of the drivers when you buy the policy, not just the ones you want to admit to.

The insurance company can either rescind the policy (cancel as never in-force for misrepresentation) or ask the insured to add the person and non-renew if they don't. They can't add the person automatically.
