How do I negotiate a settlement with the at-parties insurance that includes not only P&S, but also my medical bills ($1000) that MY medpay already paid.

I don't have MO info in front of me but I'm not sure you can recover what your MP carrier paid. If nothing else, I'd think they have a right of recovery, not you.
How do you negotiate? You figure up your medical bills, your injury, your treatment and put a dollar amount on it. This is your demand. You want to aim high but not stupid high. At that point its a... negotiation. The adjuster will offer a low amount, you will argue your case for a higher amount, so on and so forth.
