What does it mean when a health insurance policy says

I am looking in to health insurance plan through my husbands employer. There is no deductible for in network services and no out of network benefits. There are modest copays ($25-$150). For some services there is no copay amount listed and it says "subject to 100% coinsurance" Does this mean the plan pays the covered expense at 100%?

Yes, the insurer will cover 100% of the expenses for remaining part of the year up to the limits of the plan. This happens when you have completed the 'out of pocket maximum' payment obligation.
