Well..in my opinion its just a routine check-up to find whether they are in a position to cover you!
It could get them a lot of other information regarding the condition of your mouth & teeth...especially when we know how the medical bills are often mounting upon critical diseases amounting to death!
Its just a way to be sure..

Smoking affects the insurance premium to a large extent. The smokers are required to pay more premium than the average rate even for the same age base.
We applied for life insurance and i was just curious as to what information is found out through a mouth swab.
Well, normally, they checks for the smoking habits, since its an important determinant of your life insurance premium.
However, the result derived from the mouth swab can't often be trusted, as the specimen may get corrupted very soon. In that case I agree with fatman, it can only be routine check-up.
Also, if you have quit smoking long ago, then the swab test may not yield the right result, because the damaged cells might have got replaced by new cells by now.
IMO you may not have to worry a lot regarding the outcome of the swab test. It is a part of the insurance companies underwriting process.
Oral swabs are now commonly used for testing for HIV infection...plus a host of other issues. They are now routinely taken, and are much less painful that blood draws...
I have just recently asked the carrier that I work for about this very question. They told me that smoking and drugs is detected from the Urin sample only because swabs and blood tests do not show drugs or nicotine from smoking. Nicotine supposedly stays in your urin for up to 4 weeks then is no longer detected. Drugs, depending on which ones, stays in your urin longer.
