Under ERISA the company is required to provide any requested document on the insured. I f the documents that are provided contridict with other documents provided are the signs of a cover-up. Would I have a case?? Who can I speak with , in Alabama or that can help.. & Do group Universal Policies categorize everyone in the group as just group members or do they separate them labeling genders: Male & Female.. Posted: Wed Nov 11, 2009 12:49 am   Post subject:    You mentioned ERISA. ERISA only applies in the case of self-insured plans and Qualified Retirement Plans (QRPs). My feeling is that you're talking medical coverage here. Group plans through an insurer are under state (and some minor federal) laws. Are you covered under a self-insured plan through your employer?
