I was struck by a careless driver does his insurance company have to provide me with his declaration page? as well as if he has no bodily injury can i take him to court for the injuries and medical bills he caused to me or not ?

There are very few states that don't required BI.
Most states (not all) will allow you to file suit for your injuries. Depends on the nature of your injury and what state you are in. Keep in mind... you can't get something from someone that has nothing. This is why you should obtain Uninsured Motorist Bodily Injury coverage.
